Characteristics of substance Betamethasone:

Hormonal (glucocorticoid for systemic and topical use).

In clinical practice using betamethasone valerate, betamethasone dipropionate, betamethasone disodium phosphate.

 

Indications for use:

 

When applied to the skin: dermatitis (simple, exfoliative, sunny, seborrheic, radiation, herpetiformis, contact, atopic, and others.), Eczema (child, nummular et al.), Psoriasis, nodular prurigo Gaida, lichen planus, pruritus, discoid lupus erythematosus, urticaria, erythema multiforme exudative, generalized erythroderma (in the complex therapy), pemphigus, mycosis fungoides and other skin diseases.

 

Contraindications:

Hypersensitivity (for short-term systemic use for health reasons is the only contraindication).

For systemic use (parenteral and oral): systemic mycoses, herpes disease, including Chickenpox and measles (currently or recently transferred, including the recent contact with a patient), strongyloidiasis (or suspected), tuberculosis (active form in the absence of specific treatment, latent), immunodeficiency states (including AIDS or HIV infection), digestive diseases (including peptic ulcer, gastric ulcer and duodenal ulcer in acute diverticulitis, esophagitis, gastritis, recently created anastomosis of the intestine), diseases of the cardiovascular system (including the recently myocardial infarction, congestive heart failure, hypertension), diabetes (including impaired glucose tolerance), myastenia gravis, acute psychosis, renal / hepatic insufficiency, during vaccination.

For intra-articular administration, the lesion: instability of the joint, previous arthroplasty, abnormal bleeding (endogenous or caused by the use of anticoagulants), chressustavnoy fracture, infected lesions of joints, periarticular soft tissues and intervertebral spaces marked periarticular osteoporosis.

When applied to the skin: cutaneous manifestations of syphilis, tuberculosis of the skin; bacterial, viral, fungal skin diseases (including chickenpox, herpes); leg sores associated with varicose veins; skin cancer, nevus, atheroma, melanoma, hemangioma, xanthoma, sarcoma; rosacea and acne vulgaris (possible exacerbation of the disease), skin reactions after vaccination in children under 1 year — rash (diaper rash on the background).

 

Side effects:

The frequency and severity of side effects depend on the duration of use and magnitude of the dose used. High doses or prolonged use can cause GC pronounced manifestation of mineral and glucocorticoid effects, considered as a side.

Systemic effects:
From the nervous system and sensory organs: delirium (confusion, agitation, anxiety), disorientation, euphoria, hallucinations, manic / depressive episode, depression or paranoia, increased intracranial pressure with papilledema (pseudotumor cerebellum) — usually after treatment, sleep disturbances, dizziness, vertigo, headache, sudden loss of vision (for parenteral administration in the head, neck, nasal turbinate, the scalp may be due to the deposition of crystals of substance in the blood vessels of the eye), the formation of posterior subcapsular cataracts, increased intraocular pressure with possible damage to the optic nerve, glaucoma, exophthalmos steroid, the development of secondary fungal or viral infections of the eyes.

Cardio-vascular system and blood (blood, hemostasis): hypertension, chronic heart failure development (in predisposed patients), myocardial, hypercoagulation, thrombosis, ECG changes typical of hypokalemia.

From the digestive tract: nausea, vomiting, erosive and ulcerative lesions of the gastrointestinal tract, pancreatitis, erosive esophagitis, hiccups, increased / decreased appetite.

From a metabolism: a delay of Na + and water, hypokalemia, negative nitrogen balance due to protein catabolism, increased body weight.

From endocrine system: inhibition of adrenocortical function, impaired glucose tolerance, steroid diabetes or a manifestation of latent diabetes mellitus, Cushing’s syndrome, hirsutism, growth retardation in children.

On the part of the musculoskeletal system: muscle weakness, steroid myopathy, reduced muscle mass, osteoporosis (including spontaneous bone fractures, aseptic necrosis of the femoral head), tendon rupture.

For the skin: steroid acne, stretch marks, skin thinning, hyper- or hypopigmentation, petechiae and ecchymosis, delayed wound healing, increased sweating.
