Pharmacology

Mode of action — a diuretic, vasodilatory, hypotensive.

1. The drug inhibits the reverse absorption of sodium in the cortical segment of the loop of the nephron.

2. Increases urinary excretion of sodium, chloride, calcium and magnesium.

3. Reduces the sensitivity of the vascular wall to norepinephrine and angiotensin II;

4. The drug stimulates the synthesis of PGE2;

5. The drug inhibits current of calcium ions in smooth muscle cells of the vascular wall and reduces OPPS.

6. Reduces the production of free oxygen radicals and stable.

The drug is rapidly and completely absorbed when taken orally. Tmax — 2 hours after administration of a conventional dosage form (tablets, capsules). 12 hours — after receiving long-acting tablets. In plasma, at 71-79% is bound to proteins. The ratio of the blood / plasma is about 6: 1 (at the time of reaching Cmax). Ratio reduced to 3.5:. 1 through 8 h drug is metabolized in the liver. Elimination is biphasic in nature.

The drug has a dose-dependent effect.

Contraindications

Hypersensitivity, acute cerebrovascular accident, anuria or severe renal insufficiency, severe hepatic insufficiency (including hepatic encephalopathy), hypokalaemia, age 18 years (safety and efficacy have not been determined).

Limit medication when:

Diabetes in the stage of decompensation, renal failure and / or liver disease, hyponatremia, and other violations of water-electrolyte balance, hyperuricemia, hyperparathyroidism, ascites, coronary artery disease.

Side effects

From the nervous system and sensory organs: headache, dizziness, weakness, fatigue, lethargy, weakness, malaise, vertigo, muscle spasm, paresthesia, nervousness, tension, irritability, agitation, anxiety, insomnia, drowsiness, depression, conjunctivitis, violation view.

Cardio-vascular system and blood (blood, hemostasis): orthostatic hypotension, palpitations, arrhythmia, ECG changes (manifestation of hypokalemia), thrombocytopenia, leukopenia, agranulocytosis, bone marrow aplasia, hemolytic anemia.

From the respiratory system: rhinitis, cough, pharyngitis, sinusitis.

From the digestive tract: nausea, vomiting, anorexia, dry mouth, abdominal discomfort, gastralgia, constipation / diarrhea, hepatic encephalopathy (hepatic insufficiency on the background), pancreatitis.

With the genitourinary system: nocturia, polyuria, increased frequency of infections, decreased libido and / or potency.

Allergic reactions: itching, rash, hives, a hemorrhagic vasculitis.

Other: flu-like symptoms, chest pain, back pain, sweating, weight loss, exacerbation of systemic lupus erythematosus, hypercalcemia, hyperuricemia, hyposalemia, hyponatremia, hyperglycemia, hypokalemia, increased blood urea nitrogen plasma giperkreatinemiya, glycosuria.

Overdose

Symptoms include nausea, vomiting, weakness, violation of water and electrolyte balance; in severe cases — marked reduction of blood pressure, respiratory depression.

Treatment: gastric lavage, correction fluid and electrolyte balance, symptomatic and supportive therapy. A specific antidote is not known.
