Seroquel is an atypical antipsychotic preparation

Seroquel is an atypical antipsychotic preparation. Seroquel contains an active component quetiapine. It’s medicinal substance, interacting with a row the neurotransmitter receptors. The preparation blocks effect of agonist of a dopamine. Seroquel is effective for treating of positive and negative symptoms of schizophrenia.

Seroquel uses

Brand(s): Seroquel

Clinico-pharmacological group

Antipsychotic preparation (neuroleptic). It shows analogousness to serotonin 5HT2-receptors in a brain. Possesses analogousness to histaminic and α1-adrenoreceptors and is less active in relation to α2-adrenoreceptors. Doesn’t possess affinity to m-cholinoreceptors and benzodiazepine receptors. When studying extrapyramidal symptoms at animal showed quetiapine causes a weak catalepsy at influence of the dose which is blocking dopamine D2 receptors. Quetiapine reduces activity of mesolimbic A10-dopaminergic neurones compared with the A9-nigrostriatal neurones.

The preparation doesn’t cause long rising of level of Prolactinum in a blood plasma. In plenty of researches with the constant dose it isn’t taped differences in Prolactinum level at Seroquel’s use or placebo.

In clinical trials Seroquel showed efficiency at treating of all symptoms of schizophrenia.

Influence of a quetiapine on 5HT2-and D2 receptors proceeds to 12 hours after reception of a preparation.

Seroquel accept orally. A pill can be taken irrespective of food, washing down with enough drinking water. The daily dose is recommended to be divided into 2 receptions with an interval of 12 hours. Duration of treating, a dose of a quetiapine and the scheme of reception are determined by the doctor. At schizophrenia, as a rule, prescribe reception of 50 mg of a quetiapine per day (the first day of treating), 100 mg of a quetiapine (the second day of treating), 200 mg (the third day of treating) and 300 mg (the fourth day of treating). After the fourth day the dose is selected individually, considering shipping and efficiency of a preparation Seroquel. The average therapeutic dose at schizophrenia makes 300-450 mg of a quetiapine per day (an interval of doses from 150 mg to 750 mg of a quetiapine per day). At maniacal episodes against bipolar disorders at the beginning of therapy prescribe 100 mg of a quetiapine per day. The dose is enlarged by 100 mg daily within 4 days. At an inefficiency of a daily dose of 400 mg, it can gradually be enlarged to 800 mg per day (it isn’t necessary to enlarge a dose more than by 200 mg for once). Patients of advanced age Seroquel should prescribe with care. The initial dose for patients is aged more senior than 65 years makes 25 mg of a quetiapine per day. To patients with disturbances of functions of kidneys and a liver quetiapine prescribe in an initial dose 25 mg per day. At such patients it isn’t necessary to enlarge a dose more, than by 25 mg for once. Cancellation of a preparation Seroquel should be carried out, gradually reducing a dose (within not less than 2 weeks). At the sharp termination of reception of a preparation Seroquel development of a serious withdrawal is possible (which is followed by vomiting, sleeplessness, dystonia, dyskinesia and an akathisia).

Information on an over dosage of a quetiapine is limited. The reception case more than 20 g of a quetiapine without fatal consequences is described. At reception of high doses of a preparation Seroquel at patient’s development of a sedation, sleepiness, arterial hypotension, reflex tachycardia, elongation of Q-T of an interval and coma became perceptible.

The specific antidote of a preparation Seroquel is unknown. At serious intoxication carry out an intensive care, an intubation and an artificial respiration. If necessary, carry out the treatment referred on elimination of symptoms of an over dosage of a quetiapine.

It’s necessary to control a condition of the patient after reception of high doses of the preparation.

Side effects

The most frequent undesirable effects at reception of the preparation are tachycardia, disturbances of a chair, giddiness, a sleepiness, a xerostomia, the dyspeptic symptoms, an asthenia and orthostatic hypotension. From nervous system: headache, visual acuity depression, extrapyramidal symptoms, loss of consciousness, cramps, dysarthtia, syndrome of restless feet, dyskinesia. From laboratory indicators: rising of level of transaminases, hyperglycemia, depression of quantity of neutrophils, thrombocytopenia, eosinophilia, gipertriglitseridemiya, augmentation of level of gamma GT, hypercholesterinemia, kreatinfosfokinaza level augmentation.

Indications for use

Seroquel use for treating of patients with schizophrenia. The preparation Seroquel is also prescribed to patients with the maniacal episodes associated with bipolar disorders.

Seroquel is contraindicative to patients with an intolerance of a quetiapine and other components of a preparation.

It isn’t necessary to prescribe Seroquel to the patients receiving inhibitors of HIV proteases, nitrogen antifungals, and also nefazodon, erythromycin and clarithromycin (in connection with possible depression of concentration of a quetiapine).

The preparation isn’t used in pediatrics

Seroquel the patients having diseases of cardiovascular system, and also cerebrovascular disturbances with risk of hypotension need to prescribe with care.

To patients of advanced age who receive the medicinal preparations which are potentially extending Q-T interval, Seroquel prescribe with care.
